Welcome to my Counseling practice. I provide individual counseling/psychotherapy to children, adolescents, young adults and adults. My specialty area is adolescents who are struggling with anxiety, depression, school, social, academic and family issues.
I am pleased to offer my clients the benefits and convenience of an online portal. This is a secure, HIPAA-compliant website.
To begin the process, please complete the following steps:
1. Click on "Register" and fill out the personal information section.
** IF THE CLIENT IS A MINOR, please enter the client's information in the top part of the form. Enter the cell phone number for whomever should receive text message appointment reminders. (Note: you will set your reminder preferences later and you may opt not to receive text messages).
** IF THE CLIENT IS A MINOR, where it says "Bill to Contact", click on "Different" and enter the information for the parent/guardian who is financially responsible.
2. Verify your account.
(You will receive an initial password via e-mail. When you log in during the verification process, you will be prompted to change your password.)
3. After verifying your account, you will automatically be taken to the Client Information Form. Please complete and save this form.
4. After completing the Client Information Form, you will automatically be taken to the "Policies and Practice Information" form. Please review this form and indicate on the last page that you have reviewed it.
** IMPORTANT: This document also serves as the consent form. I ask that you carefully review this document before we meet so that I can answer any questions you may have.
5. If you plan to submit to your insurance company, feel free to click on the "Files" icon and download the "Questions to Ask Your Insurance Company" form. This is intended solely as a helpful guide when contacting your insurance company to determine your eligibility benefits.
6. Finally, you may want to add the following e-mail address to your contacts list / address book to decrease the chance that e-mails from the secure portal get sent to your spam box: https://colleendesmond@securepatientarea.com
